I took mirtazapine for a while as an on-demand medication for sleep disorders (difficulty falling asleep) and it worked very well for years to bridge stressful phases. It makes you very tired at the beginning and you fall asleep well. I didn’t have a bad overhang effect.
I deliberately took mirtazapine (it was only supposed to be an on-demand medication for me to sleep, as I already take an antidepressant in the morning) for about 2 weeks at a time in order to be able to monitor its influence on my sleep.
Unfortunately, mirtazapine totally depressed my heart rate variability (HRV) (which was always measured at night by Polar). My value was only around 36, and that as a trained athlete…
In addition, my ANS status (ANS status = recovery of the autonomic nervous system) was always very (!) poor under mirtazapine.
My heart rate also increased at night from the usual 45-48 bpm to 55-58 bpm.
I didn’t notice much of this while I was sleeping, but during the day I was very restless, even overexcited. My night-time values were simply frighteningly bad…
I sleep similarly well with another medication (which I am currently taking).
However, it does not lower my HRV / HRV. Nor does it increase my heart rate.
I think it’s important that something like this is checked (it’s different for everyone and doesn’t necessarily affect everyone).
After all, a low HRV / HRV is detrimental to health in the long term.
Apart from that, I found Mirtazapine very well tolerated.
It just made me really hungry (especially in the evening). But as I tend to be someone who doesn’t put on weight from eating anyway, I didn’t really care.
Overall, however, mirtazapine is supposed to be a very good remedy.
However, the heart and autonomic nervous system thing (in my case) really bothered me.
That’s why I now take a different medication (a tricyclic AD) for sleeping.
As I said, every body is different. Just because it was like that for me on mirtazapine doesn’t mean it has to be like that for other people.
As part of my Long-Covid therapy I had to start with mirtazapine 15 mg. The indication was not depression but the effect on the neurotransmitters. After 2 weeks, the dose was increased to 30 mg. Within a very short time, I gained 22lbs in water retention. The suffering was enormous. Although first reduced and then stopped, I have been struggling to get rid of the water for 2 months. Despite lymphatic drainage, drainage teas, herbal substances, Lasix and Apis globules, I was only able to get rid of 6lbs again. The doctors are at a loss and think it will just take a lot of time.
Unfortunately sexual side effects at 30mg. Reduced sexual function and libido. After discontinuation, the former returned within a few months. Libido still greatly reduced. Sexual dysfunction in the sense that the erection becomes weaker is very rare, I was simply unlucky. However, I doubt that it is the same with libido; patients are not listened to here anyway because psychiatrists always blame it on depression. Mirtazapine acts on all kinds of receptors, so that shouldn’t come as a surprise. There will probably be many cases where everything works downstairs (even in people who would have reacted sensitively to SSRIs) but the libido falls by the wayside and it is difficult to say whether this will come back… I can imagine that the number of unreported cases is not necessarily much lower than with SSRIs and that a completely false picture is being conveyed.
Be that as it may, it worked quite well. But the psychotherapy was certainly the more decisive element for me. And it led to a certain sagging problem, I made the most progress after stopping it. I wish I had been given bupropion.

I have been taking 15mg of mirtazapine every evening for over six months because of my insomnia. I am very satisfied with the active ingredient and have no side effects at all. No weight gain either. I can only recommend it, it lets you sleep well and the next morning there is absolutely no overhang.
After taking it I was practically in a waking coma. Total tiredness, concentration problems, unable to think clearly, brain fog
Particularly during the first few days I felt very dizzy and had severe headaches. In addition, often a bloody nose and occasional nosebleeds.
Clinically prescribed years ago. Unable to get up at night, heavy overhang for a long time during the day. Already increasing hair loss. Circulatory collapse when getting up at night. Therefore discontinued the 1st time.
Opipramol, trimipramine, quetiapine tried, even more side effects, sleep never sufficiently restful in relation.
Tried mirtazapine again and again over the years in varying doses, as hair loss was the “only” side effect at the beginning. Sleep effect, quality was improved at the time. Weighing up what was more important became increasingly difficult, as the hair loss caused by both the medication and the sleep problems led to additional depression.
Each time I tried again, more side effects, higher dose needed for sleep. Unfortunately, an endless cycle.
Experience with 15mg and 30mg
I last weighed 48kg at 176cm. The reason for this was the unhealthy relationship, I had extreme fears of my ex.
My doctor prescribed me 15mg to help me sleep better and gain weight. Both worked really well. I was able to sleep again and had an appetite. At first it was enough to take the tablet an hour before going to bed, but at some point it was no longer enough and I had to take it even earlier. Then another aggravating event occurred and I switched to 30mg so that the main focus was no longer on sleep but on having a positive effect on the psyche. I don’t recognise myself. I’m tired all the time, have a shitty attitude and thoughts of ending my life, circulatory problems and the impression that I have water retention and extreme headaches. I have only been taking the 30mg for 9 days and have decided today to stop or split the tablet to get to 15mg or take only 1/4 to improve sleep only. It has been proven that the lower doses improve sleep and the higher doses of 30mg and above affect the psyche. A lot does not help much.
I have been on 15 mg since February and it has helped me a lot with eating, weight and sleeping well. But was not good with the stomach fast waves anxiety, stinging many possible just like with 30 mg.
But helped me very well.
Bad risk getting venlafaxine worst stomach pain, headaches not a good deal.
I’m curious I get 45 mg tonight then I think good helps against panic attacks
Appetite stimulating effect depending on dosage from slight to stronger.
Discontinuation symptoms: Dizziness and drowsiness.
I have been prescribed the tablets in combination with another antidepressant in the morning. I take them in the evening around 10 pm and sleep like a rock. No brooding, no inner restlessness. I close my eyes and just fall asleep.
In the first week and a half, I had very restless legs in the evening, two hours after taking the tablets, and after consulting the doctor I stopped taking them for the time being. But then the insomnia started again immediately and I started taking the tablets again.
Sleep was just so important to me and as it has more benefits for me than side effects, I decided to continue taking them. In the meantime, the restless legs have stopped and I’m just fine with them.
Occasionally I have an increased appetite, especially after taking it, but I have that under control quite well.
I don’t usually write reviews, how could I? Have been torturing myself with severe depression and severe anxiety disorder for no reason since the birth of my 1st child (6 years now). No matter which AD we tried, either they didn’t work AT ALL or one that worked after months just made me numb. I am 29 and have already tried about 8-9 ADs (citalopram, escitalopram, venlafaxine, duloxetine, bupropion, sertraline, fluoxetine). In general, I had a catastrophic reaction to the SSRIs and SNRIs! Massive aggravation, off track, massive anxiety etc. Mirtazapine is the first and only medication that works for me and is effective! For both anxiety and depression. I am now improving and on the right path. So it’s a lifesaver for me. The bad thing is that it leads to water retention quite quickly, even though I’m not that inclined. Nevertheless, it’s definitely worth it! I wasn’t capable of anything before and couldn’t lead a life. So what good is a great figure without a normal life!
I experienced severe side effects, including swelling of the hands and feet, breathing difficulties, and an extraordinary surge in hunger, as if I was famished! Additionally, I felt excessively drowsy during the day. In the brief period that I was on this medication, I put on approximately 10 lbs.
I used this for depression, but it only resulted in weight gain and improved sleep. It had no positive impact on my mental health. I discontinued its use after six months without experiencing any withdrawal symptoms. It appears to me to be nothing more than a sleep aid.
